Welcome and thank you for making an appointment with Preferred EAP. To expedite your registration, please print each of the forms below, complete them, and bring them with you to your first session. Complete a full set of forms for each person to be seen in session.

Services provided by Preferred EAP are free of charge to eligible participants. However, if you fail to keep an appointment or cancel an appointment with less than 24 hours notice, that session may be deducted from the sessions for which you are eligible. If you need to cancel an appointment with us, please call 610-433-8550 well in advance.
