Leah Posivak-Khouly, MPH, CHES®

Manager, Condition Support

Leah provides oversight of the Condition Support/Disease Management program team. The Condition Support program provides clinical health coaching to improve the quality of care and health outcomes for participants living with Asthma, Diabetes, Hypertension, Chronic Obstructive Pulmonary Disease, Congestive Heart Failure and Coronary Artery Disease. She received her bachelor's degree in Biobehavioral Health from Penn State University and her master's degree in Public Health from East Stroudsburg University.
